| Description: | One of the biggest complaints that patients have about going to see a doctor has to do with the waiting time and the paper work. You should know that we are not too thrilled about those problems either. Here are some ways to save you some time, and to make your visit with us more pleasant, informative and helpful to you. In order to save you some time, please download our registration and medical history forms at home before coming in to see us. You may fill them out ahead of time, at your own convenience so that you won't have to bother filling them out when you get here. Once you have downloaded the forms, you may fill them out on your computer, and print them out. Please remember to bring them with you to your appointment. If you have seen another doctor for this problem, please bring any x-rays, CT scans, MRI's, operative reports, photographs, and medical records with you. If you are being referred to Dr. Katz by another doctor, please bring a copy of the referral form with you. If you have trouble remembering the names and dosages of your current medications, please write them down on a piece of paper and bring the list with you. Or, you may fill out our medical history form which has a section that will ask you about your current medications. If you have difficulty speaking English, trouble hearing or seeing, you will do yourself a huge favor if you bring someone with you who can interpret for you. If you are having a problem moving, walking, standing or anything like that, try to bring someone with you who can help you. Remember, you should not be driving if it is not safe for you to do so. If you are having a foot problem, and tend to wear a particular type of shoe (especially if you are an athlete), please bring your shoes with you. |
